This 32-year-old mother desired breast enhancement surgery. During his examination, Dr. Steele noticed some mild constriction of the lower breast pole with early nipple ptosis. He was concerned that a standard augmentation with submuscular placement would further push the patient’s nipple downward. After discussing the pros and cons of various options, the patient and Dr. Steele decided that placement of a textured implant above the muscle with a conservative peri-areolar breast lift would give her the best result. Dr. Steele used a Gore-Tex suture to prevent expansion of the areolar scar and her scar is almost imperceptible 4 months after surgery. She is very pleased with her new size and upper breast pole fullness.
